    (E-169) Demonstration of navigational performance of preclinical angioscope designs in neurovascular phantoms

    Introduction:: The role of mechanical thrombectomy for treatment of ischemic stroke is an ever evolving field of study. Although fluoroscopy, magnetic resonance imaging and computed tomography angiography are the conventionally used imaging modalities to guide mechanical thrombectomy, they are limited by their resolution and sensitivity. A forward viewing angioscope (optical imaging catheters) that is able to produce full color, high resolution video of the endovascular intervention is a possible solution to this clinical need. However, these angioscopes are currently limited by their ability to reach intracranial targets.

    Materials and Methods:: To test their accessibility, glass phantoms of the internal carotid artery and middle cerebral artery bifurcation, representing a range of clinically analogous anatomy were used. Different lengths of a rigid tube were then inserted into the distal end of a microcatheter to simulate the various sizes of the angioscope and navigated into the phantoms to help understand the effect of the rigid tube length on accessing distal vessel targets.

    Results, Conclusions, and Discussions:: For the simple anatomical phantoms, a rigid tube length of 2.5 mm and 5.0 mm provided the least challenging navigation. For the complex anatomic phantom, a rigid tube length of 2.5 mm provided the least challenging navigation. Future designs of forward viewing angioscopes should aim for a rigid tube length of 5 mm or shorter to achieve accessibility to the middle cerebral artery (MCA) bifurcation.
